SOLEI has more Google reviews, 93 against 63. Both are rated 4.9.

We track 28 GLP-1 search terms in this market. Bay Area Medical Group appears in the Maps results for 27 of them. SOLEI appears for one.

Reviews are not the entry ticket

Google says local results are based on relevance, distance and prominence. Prominence is the one most operators work on, because reviews are its visible part.

Ten of the 44 clinics have no reviews recorded. Across the 34 that do, review count told us nothing about how many searches a clinic appeared for. The clinics with the most reviews were not the ones showing up.

The extremes make it concrete. Be CLINICAL has 350 reviews and appears in none of the 28 searches. Premier Physique has 22 and appears in 27.

Reviews are not useless, but in this market they did not predict Maps visibility. Our reading is that reviews help rank a business once Google already treats it as relevant to the search, which would explain why Be CLINICAL's 350 reviews coincide with no visibility at all. That is our interpretation of the pattern, not something we tested directly.

Category predicts how many searches a clinic appears for

The difference between those two clinics in the same building is the primary category. Bay Area Medical Group is Weight loss service. SOLEI is Medical spa.

Read the reviews column downward: 22, 63, 69, 359, 72, 112, 93, 350. The two most reviewed clinics in the market sit fourth and eighth. Whitespark's 2026 survey of 47 local search experts across 187 factors puts primary category as the top individual factor for the local pack. This is consistent with that.

A second pair says the same thing 331 feet apart on Crow Canyon Place. Abundant Life Weight Loss is Holistic medicine practitioner, 69 reviews, appears for 25 searches. Be CLINICAL is Medical spa, 350 reviews, appears for none.

One warning for anyone auditing this at scale. We got it wrong at first because our tool read categories from the Google Places API, and Places types are a different taxonomy from GBP categories. Neither Weight loss service nor Medical spa exists in the Places list. Both return spa, so these two clinics look identical in Places data. Places API (New) does not fix it. Every category above was read off the live listing by hand.

When the category cannot change

Most clinics run more than one service line and cannot hand their primary to weight loss.

Google's own guidance is to pick categories that complete the sentence "this business is a," not "this business has a," and to use as few as possible. A long list of secondaries describing every service offered works against that guideline rather than with it. Choosing a specific category makes Google imply the broader ones anyway.

Changing the primary is reversible at any time. If weight loss has become the revenue line rather than a sideline, that is worth testing rather than assuming.

A second profile is narrower than it looks. Google permits departments to have their own profiles, but they need a different name, a distinct category and typically a separate customer entrance. A hospital's dermatology department qualifies. A section inside a larger store does not.

The address caps how far a clinic can win

Premier Physique and Bay Area Medical Group are both Weight loss service, so category is held constant. They sit about ten miles apart.

Eleven near-me searches, measured from five city centres:

Each is close to first at home and falls away with distance. Premier Physique averages first in Pleasanton and fourteenth in Danville. Bay Area Medical Group is the mirror image.

Neither clinic wins this market. Each owns two or three cities from one address, and no profile change alters that. Anyone selling Maps visibility across a whole region from a single location is not mentioning this constraint.

Maps and organic are separate systems

MedLounge appears on page one of regular Google results for 72 search terms and in Maps results for 9. Bay Area Medical Group is the reverse: 25 organic, 40 Maps. Same city, similar review counts.

They read different inputs. Maps reads the profile: category, services, address, reviews. Organic reads the site: pages, content, links. A business can be strong in one and absent from the other, and fixes do not transfer. Telling MedLounge to publish more content would be wasted advice. Its content already works. Its profile is what costs it Maps.

Where to start

For anyone auditing local clinics: check where the category data comes from before trusting any relevance score built on it.

For anyone running a clinic: open the listing and look at the grey line under the star rating. That is the primary category and it is public. Read the same line on the three clinics above you in Maps for your main search term. If they do not match, that is the first gap to close.

We have not yet watched a clinic change its category and tracked what followed, so we cannot say it will move you. What we can say is that in this market the clinics appearing for weight loss searches are categorised differently from those that are not, and the pattern held in two pairs close enough together for distance to be irrelevant.

One more thing. Maps leads here because it is the only channel anyone in this market has invested in. Local Google Ads spend across all 44 clinics is currently zero, and organic and AI visibility are close to untouched. Category work opens the Maps door. It does nothing for the other three.
