Here is what the competitive picture looks like.


The Tri Valley GLP-1 market

An estimated $15.9M in GLP-1 patient revenue flows through Tri Valley every year. That works out to roughly 378 potential patient contacts per month across all 44 clinics we audited.

Both figures are modeled estimates. We calculate patient capture from tracked near-me search volume, weighted by each clinic's presence across Maps, organic, and paid search. Revenue uses a $3,500 average GLP-1 patient LTV. These are upper bounds. They don't discount for national telehealth providers who compete for the same demand. The full methodology is here.

What the model captures accurately is the structure of who gets what. And that structure is stark.


The split is severe

Three clinics capture 40% of all estimated patient contacts in the market: Premier Physique in Pleasanton, Bay Area Medical Group in San Ramon, and Abundant Life Weight Loss in San Ramon. Combined, roughly 154 patients per month go to three practices out of 44.

Twenty-three clinics capture zero. Not low. Zero. Invisible on every channel we track.

The remaining 18 clinics share about 224 patients per month between them, roughly 12 each.

This is the pattern we see in most local GLP-1 markets. A small number of clinics capture a disproportionate share. The majority get very little or nothing.


Five channels, five different stories

We score clinics across five dimensions. In Tri Valley, each one tells a different story.

Google Ads. Zero local clinics are running ads. Not one. There are 9,428 monthly searches for GLP-1-related terms in this market. The auction is not empty. National telehealth providers are already bidding. But no local clinic is competing for this demand yet.

Google Maps. This is where most of the patient capture happens. Premier Physique and Bay Area Medical Group each appear in 11% of tracked near-me searches. Abundant Life is at 10%. Twenty-six clinics don't appear in the Maps pack at all for these searches.

Organic search. Thin across the board. MedLounge ranks on page 1 for 10% of tracked keywords. LA ViE MD and Bay Area Medical Group cover 7% and 3% respectively. Most clinics don't appear on page 1 for a single tracked term.

AI visibility. LA ViE MD Danville is named in 44% of patient queries we ran across ChatGPT and Gemini. MedLounge is at 40%. Twelve of 44 clinics are never cited across 50 queries. Note: AI citations are not yet factored into our patient capture estimates. The channel is active and the distribution is already unequal. We are watching closely how it converts to actual bookings.

Online presence. Review counts range from 0 to 359 across the 44 clinics. Reviews correlate more strongly with AI citation share than with Maps presence. A clinic can have strong reviews and near-zero Maps visibility at the same time. Reviews build authority. They don't guarantee visibility on their own.


What the data actually shows

The top three clinics on patient capture share one thing right now: Maps presence. Premier Physique, Bay Area Medical Group, and Abundant Life all appear in the Maps 3-pack for the highest-volume searches in the market.

But this needs context. None of the top three run ads. None lead on organic search. None dominate AI citations. Maps is where they win because it is the only channel anyone in this market has seriously invested in. The other four channels are largely untouched.

That creates a specific opportunity. The clinic that starts running ads competes against national telehealth, not local peers. The clinic that builds organic coverage for the top five search terms faces almost no local competition. The AI channel is already concentrating. Our market report shows 70% of patient queries name at least one of the top 3 clinics. The remaining 41 clinics are splitting the rest, or absent entirely.

Our read: Maps presence is the baseline. Every clinic that wants to capture patients needs it. But the growth opportunity across this market is in the channels nobody has touched yet. Ads, organic, and AI are open. The question is who moves first.
